For additional information about how to use the forms, see the comprehensive instructions preceding Forms AB-100, AB-101, and AB-200.
- Forms AB-100 and AB-101 have updated instructions. No changes were made to the contents of the forms.
- Form AB-200 has been revised to allow for a more streamlined renewal process that does not require licensees to provide new Forms AB-100 and AB-101 each year if the paperwork from the previous license year is still accurate.
- Form AB-200 now includes Appendix A, which creates a standardized listing of all persons involved in the applicant business and an easy way to identify when personal information has changed.
